**Soulmate Club referral program terms and conditions:

  1. Tiered Prizes and Reset: The Soulmate Club includes two tiers of prizes. Patients have the option to claim their prize once they reach three completed referrals, or they can choose to continue accumulating referrals to reach the five-referral tier for a larger prize. Upon claiming a prize at either tier, their referral count will reset to zero, and they can begin accumulating new referrals.
  2. Referral Count Time Limit: The referral count has a one-year time limit. After one year, the count for a given patient will reset to zero, regardless of whether a prize has been claimed. The one-year period for tracking referrals will commence from the date of the first referred patient’s completed appointment.
  3. Prize Flexibility: The prizes currently posted may remain the same, or they could be updated occasionally, seasonally, or permanently to reflect new offerings or promotions.
  4. Referred Patient Incentive: The new patient that is referred by an existing patient will receive $25 treatment credit.
  5. Qualifying Conditions for Referred Patient Count: A referred patient that books and completes their visit at one of our Soul Dental clinic locations is counted as 1 referral. Referred patients must list the referring party in their online booking form, reference the referring party during their phone booking, or present their paper referral at appointment check in, and the referring patient must be an active patient.
